Case Managers who are Registered Nurses will assess potential clients to determine eligibility for services and arrange for individualized care. A variety of services may be provided within the guidlines of the program. An example of some of the services which are available are: Personal Care, Homemaker, Adult Day care, home modification, Emergency response system, meals, transportation, equipment, shopping assistance, Caregiver Respite, and overnight Respite.
